Many individuals who have a confident hepatitis C virus (HCV) antibody (Ab) test never obtain a confirmatory HCV RNA viral load (VL) test. Reflex VL assessment can help deal with this dilemma. We undertook a systematic review to judge the effectiveness of reflex VL testing compared to standard nonreflex methods on outcomes over the HCV care cascade. We searched 4 databases for studies that examined laboratory-based response or clinic-based response VL screening approaches, with or without a nonreflex comparator, and had data regarding the uptake of HCV RNA VL test and therapy initiation and recovery time taken between Ab and VL testing.
